Dimension CRM allows you to create and deploy lead generation forms easily.

Click on Contacts Tab and then click on Contact Form link item.

Tick the checkboxes next to the fields you wish to include inside your lead generation form.

Success URL : This is the page everyopne will be sent to once they fill the form. E.g www.mywebsite.com/thankyou.html

Failure URL :  This is the page everyone will be sent to if there was a problem processing the form. E.g www.mywebsite.com/error.html

After this, you can create the HTML content for the email notification that comes back to you each time the lead form is filled on your website.

You can also automatically send an auto response back to whoever filled out the lead form as a "Thank You" note. If you leave this blank, no notification email will be sent back to the person who filled the lead generation form.
